Release Forms Can't Post

The bride who's engagement pictures I shot is asking me for a release form so she can have her programs using some of the pictures I took printed at Staples. Does anyone here have such a form they can email me or know where I can get one? She said that Staples will ask for one if the pics look even close to professional.

Walmart recently asked for a release from a client, and I searched for "Copyright Release" to view a few.... I did find several from web-based printers and various image users.

FOR THE SHORT TERM:
I'm no lawyer, but In the end I just drafted this, signed and sent it to her:

Copyright Release Form


Jeko Video (photographer) does hereby authorize our client indicated below to make any reproductions of the image(s) from their event. I understand that the client will use this image to create one or more reproductions and may alter the pixels in the original image(s).

You will need to identify the client, add the date and sign underneath. Also, your contact information should be on there somewhere (it is in my business letterhead). If I ever hear from someone who won't accept this, I will ask them what it needs then and adjust.

IN THE LONG RUN:
I am trying the integrate this directly onto my photo-disk itself, which I will then also sign with a soft marker. I figure they have all they need with them that way whenever they go to print it.

Mine's not too much different:

Please be aware that in accordance with their contracts, my Clients have purchased the high resolution digital files from their wedding photography. They have permission to make a unlimited number of copies and/or reprints of any images, or to reproduce the images for any non-commercial purpose, for personal use at no charge.

Wal-Mart has an agreement that you can fill out on site and they'll keep it on file. I did this for the high school when I shot their musical. Anyone with a CD could go in and print out the photos.

I think most places just want something to cover their butt and aren't going to get overly concerned about the wording of the release.

I, too am thinking about printing the release on the disk. I know of at least two other photographers that do the same thing.

I am concerned about signing with a pen, and will use a marker to avoid damaging it.... am I being overly concerned??

Ya know ... I think they do. However ... I've provided a couple of clients with the written release and that still might be the best way to go. I'm going to encourage people to make a backup of their CDs in case something should happen to theirs. While I do keep a copy of the CD I don't want to be held responsible for having one.

I guess the point is that if they make a backup then its not going to have the release on it should they actually have to use it.
