Resource for photo contracts/agreements?

Are you writing your own contracts/agreements for customers to sign for shooting events? I'm basically putting together a standard sales agreement, saying I provide X number of hours of coverage for $X per hour. But I guess it will need to state things like ROUGHLY how many pictures they will receive, how/when the pics are to be delivered (i.e., immediately following the event/or within 24 hours of the event in case a disc can't be burned on the spot), rights of use (i.e. all commerical/personal rights of the pics belong to the customer, photog won't use them for public display or personal profit), etc, etc.... For non-paying gigs, I've got a generic pad of model releases. But for paid event work, I'd like to have the proper forms to cover my butt, instead of the Frankenstein contract I've created for cheap wedding videos. Any thoughts, examples, resources, suppliers, links, etc, appreciated!
