Does anyone know about how new satellite TV channels get started? What is the business structure (i.e., who pays for the channel, etc.)?

Commerce comes in many shapes and sizes and even gets creative from time to time. For example... Anheuser_Busch has their own satellite shows but opts to host an uplink in exchange for riding piggyback.
Anyone with a feed may be open for negotiation. As with everything else, the best deal offered isn't the best deal. Haggle.
