obtaining movie rights Can't Post

Does anyone know where I can find a standard form that I could use to obtain movie rights? It concerns a story I want to do about someone's life. The person is no longer alive, but his family is, so I need to contact them. I would appreciate your advice!

Does anyone know where I can find a standard form that I could use to obtain movie rights? I have written a true story of someone's life and I have a producer who is interested in it, but I must first acquire the rights from the subject. Any help you can give me would be greatly appreciated.
